how to insert image in CAPI like text item .

please see this documentation for the details on how to Add an Image to your CAPI question text. Images can be inserted at the top in the CAPI question text, or used in value sets, but not inserted below the questions on the form.

I mean, i want to have a static simple image for the head of my questionnaire not related in question, below it, there are questions. is this possible and how i can do it(i really have to do this, i spent a lot of time without solution) ? your link maybe explain how to have image in answer of question

Currently, it is not possible to insert an image directly on the form.

Maybe i'm wrong by using the word "insert", i will use "Add" or show an image in the form. is it possible?

No, it is not possible. The form can only contain question fields. You can add an image to the question text, or to an HTML popup window, but not to the form itself.
