now suppose the selected response as ( 1-HA, 2-ANM, 5-NURSE) ....FOR OTHER ( #AHW, # LAB TEC)

q2) Avaliability of service in the health facility? Q3) WHO PROVIDE THE SERVICES? (FOR EACH SELECTED SERVICE, CHOOSE THE SERVICE
PROVIDER?
1) OPD 1) HA
2) DENTAL CHECK-UP 2) ANM
3) EYE CHECK-UP 5) NURSE
4) IMPLANT #) AHW (OTHERS RESPONSE)
5) IUCD #) LAB TEC (OTHERS RESPONSE)

SO, FROM THE ABOVE QUESTION (Q1) IS IT POSSIBLE TO SHOW THE SELECTED RESPONSE ONLY IN THE (Q3). SO, THAT THE CHANCES OF ERROR WILL BE LESS. IF IT IS POSSIBLE CAN WE CONVERT THE OPEN ENDED RESPONSE AS A OPTION, LIKE IN (Q3) SHOWN ABOVE????. please let me know about it, if anyone have any idea regarding this issue please comment on it.

You can do this using dynamic value sets (setvalueset). A lot of similar questions appear already on this forum with solutions.
